Patient Care Coordinator – Heart & Lung Clinic

About the role

The Patient Care Coordinator serves as the primary point of contact for patients at the Heart and Lung Clinic, handling test results, lab and radiology orders, and procedure scheduling. Working closely with providers, the coordinator ensures patients receive accurate information and timely support throughout their care journey.

Key responsibilities

• Schedule radiology and laboratory procedures, obtain insurance authorizations, and complete required paperwork.
• Communicate test results to patients, discuss abnormal findings with providers, and arrange follow‑up visits.
• Provide pre‑procedure education, including preparation, risks, benefits, and cardiac clearance when needed.
• Review medications with patients and ensure all consent and compliance forms are signed.
• Coordinate referrals to specialty services and act as liaison between providers, pharmacies, and external physicians.
• Maintain detailed patient records of procedures, communications, and outcomes.
• Follow up post‑procedure to address concerns and relay provider recommendations.

Required profile

• High School Diploma or GED (required); Associate’s degree preferred.
• Completion of a Medical Assistant training program.
• At least 1 year of experience in a medical office or patient‑care setting (preferred).
• Basic Life Support (BLS) certification.

Required skills

• Basic Life Support (BLS) certification.
• Knowledge of insurance authorization processes.
• Strong patient communication and education abilities.
